Helping You Start Off on the Right Foot
Looking forward to your first visit to Malecki Family Dental? So are we! It’s always exciting to meet new patients for the first time, and we want the initial appointment to be a success. On this page, we’ve outlined the information that you absolutely need to know before you arrive at our office. Is there anything you’ve been concerned about that you don’t see addressed on this page? Please call and let us know so that we can clear up your doubts ahead of time.
Your First Visit
We ask that you arrive a little early to your first appointment so that you can make the necessary finishing touches on the requisite paperwork. Once you have been called back to the treatment area, a thorough examination will be performed, X-rays will be taken, and plaque and tartar will be removed from your teeth and gums. We may recommend specific follow-up treatments if we found symptoms that point to potentially severe oral health issues.
New Patient Forms
If you don’t want to spend too much time in our waiting room, you can click the button below and download all of the necessary forms right now. By filling them out before you arrive for your appointment, you’ll be ready for your visit when you walk through the door, meaning we’ll be calling you back to the treatment area all the sooner. Of course, if you need help completing these forms, you can still call our office for advice.
Dental Insurance Welcome
Paying for dental care is stressful enough without having to worry about filing insurance claims or making sure that you’re getting the most out of your dental plan. Our team can handle your insurance paperwork for you, and they can discuss the benefits available through your specific insurance carrier. Don’t forget to ask if our practice is in-network with your insurance carrier or to voice any other concerns you might have before you arrive for your first appointment.
Financing Available
Paying a single lump sum for dental care can take its toll on your wallet, but third-party financing can make things much easier. We work alongside CareCredit in order to offer patients low-interest financing plans that let them pay for care in monthly installments. Almost any dental procedure can become affordable with the right CareCredit plan. Click the link below to apply today, or call our office to learn more about how this option works.
